Bay Cove Human Services, Inc a leading Massachusetts human services organization, seeks an Assistant Director of Care Coordination who will create Care Teams that will deliver care coordination services to MassHealth members.
The Assistant Director of Care Coordination partners with the Director of Care Coordination to provide oversight of Behavioral Health Care Coordination as implemented through CP Care Teams in the Greater Boston, Southern and Northern Regions.
S/he is responsible for the development and implementation of care team policies, procedures and work flows so as to ensure that each Care Team is meeting quality performance metrics and Enrollee satisfaction.

You will:
Oversee Care Coordination as implemented through CP Care Teams- develop curriculum, team policies, procedures and workflows.
Hire, train, supervise and evaluate Team Leaders and LTSS Care Coordinators.
Liaise with Team Leaders in developing and maintaining ongoing working relationships with Enrollee’s ACO/MCO plans.
Collaborate with Community Care Partners (CCP) team to analyze program data and implement quality improvements as indicated.
Solid understanding of the healthcare environment- experience working in health care or community-based organizations doing project, program or contract management work.
Experience- working with people living with SMI and/or SUD Track record- hiring, supervising, and managing high performing teams.
Education- Master’s Degree required.
Licensure- License to practice as “Licensed Practitioner of the Healing Arts” in MA; or MA license to practice independently (LICSW, LMHC) preferred.
Problem solving skills- demonstrated experience in evaluating all aspects of a problem or opportunity and make or facilitate appropriate and timely decisions.
Effective communication skills- ability to facilitate, exceptional customer service; organizational skills.
High degree of social facility in obtaining cooperation and support from a broad range of people.
Ability to work collaboratively- within an organization and with leaders of external organizations.
Ability to articulate and communicate the Community Partner program’s mission.
