The Veterans Administration Cheyenne Health Care System is seeking candidates to fill one position for a Social Worker-Program Coordinator - Rural PACT position with a not to Exceed time limit of 1 year. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. Grade Determinations: To qualify for the GS-12, Social Worker Program Coordinator must meet the following criteria in addition to the Basic Requirements listed above: Experience and Education: One (1) year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgement, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities (KSAs): In addition to the experience/education and licensure/certification above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Point of contact for the Office of Rural Health's PACT Social Work initiative in the rural CBOC and Mobile Telehealth settings. Facility subject matter expert in the standard of care for social work in the rural PACT setting. Ensuring that PACT social work is fully integrated into the interdisciplinary treatment team and works in collaboration with PACT providers, nurses, and other disciplines to ensure effective communications and teamwork. Promoting the care coordination and care management functions of social work in the PACT setting. Assisting with the integration of Whole Health into the practice of PACT social work and the PACT interdisciplinary team. Providing social work services to include high-quality assessments, case management, clinical treatment, advocacy, support and care coordination, referral and resource management. Working in a remote setting as the sole social work practitioner without access to Social Work Supervision or other social workers on site; must utilize advanced skills, judgement, and independence developing services and determining clinical outcomes and interventions. Overseeing daily operation of ORH grant funded program, developing SOPs for program operation, developing relationships with providers and community partners to promote PACT social work in the rural areas. Preparing reports and statistics for facility, VISN, and national use. Maintaining compliance with Office of Rural Health and National Social Work guidelines for documentation and intervention using the PACT framework. Adhering to the NASW Code of Ethics and all local social work policies. Developing effective partnerships with local, state, federal, and non-governmental agencies. Independently assessing psychosocial functioning and needs of Veterans and family members. Providing full range of social work services. Providing interventions independently with Veterans and families/caregivers experiencing a wide range of complicated problems. Providing a range of interventions and treatment modalities. Providing case management and care coordination. Participating in care planning with interdisciplinary team. Providing consultation/education to Veterans and caregivers concerning VA and community resources, benefits, and support services. Acting as consultant to other team/staff members, providing customer service, advocacy, and education to internal and external customers. Acting as liaison between VA and community resources. Acting as subject matter expert for Advance Directives, Organ Donation, and Advanced Care Planning. Participating in committee assignments, quality improvement projects/practices including peer chart reviews, data collection, and monitoring customer satisfaction. Assisting Veterans and caregivers in coping with loss and grief related to disability, chronic and terminal illness, and death. Documenting social work interventions and activities in Veteran's clinical record. Linking Veterans with services, resources, and opportunities to maximize Veteran's independence, health, and well-being. Conducting timely assessment of at-risk Veterans in crisis. Providing subject matter consultation to colleagues and students on psychosocial treatment of Veterans. Attending staff meetings and other mandatory meetings.
