Atlantic Health System is seeking a per diem Social Worker for Case Management Department.

This position is per diem with flexible hours including both week-day and week-end/Satuday coverage needed.

• Consultant to healthcare team
• Resolution of crucial problems of patient/family
• Assist with adjustment to illness issues
• Facilitate formulation of post-acute care transition plan

Education:

• 
  MSW with current LSW or LCSW license in the state of New Jersey

Relevant Experience:

• 
  At least 1 year of acute care experience; knowledge of systems--financial, insurance, advocacy, legal; knowledge of community resources
