Christiana Care is looking for a fulltime Licensed Social Worker for the Holloway Community Program.
The Holloway Community Program at ChristianaCare was established in October of 1989. Since its inception, the Holloway Community Program has been the major care provider for people with HIV/AIDS throughout the state of Delaware.
The Holloway Community Program has a staff of 51 professionals, including five board-certified infectious-disease doctors, board-certified internal-medicine doctors, an OB/GYN provider, nurses and nurse practitioners, pharmacists, a psychiatrist and behavioral health consultants.
This position is a split position, rotating between the Smyrna and Georgetown offices.
PRIMARY FUNCTION:
To facilitate and coordinate care of patients (adolescent, adult & geriatric) infected with HIV, HCV and other infectious diseases. Works to assist patients and their families to adjust to personal and environmental difficulties which predispose illness or interfere with medical care
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Maintains a current knowledge base of HIV disease process. Utilizes knowledge to appropriately assess patient needs, and implements or facilitates appropriate intervention. Works appropriately with health care team to coordinate care on an outpatient basis
Evaluates and provides psychosocial support to patients and their families.
Assists patients in accessing appropriate supportive services in the community
Maintains current knowledge of resources available to assist other health care providers and HIV program staff in coordination of care
Collaborates with DPH case managers and community agencies to provide a comprehensive continuum of patient care and to maximize utilization of available resources
Conducts financial screening for Ryan White, ADAP, ADAP co-pay services in keeping with established organizational and Division of Public Health policies. Assists eligible patients in enrollment into Delaware Medicaid or private insurance through the ACA Marketplace. Utilizes MAGI process to determine and document current FPL level.
Assists in recruitment of patients into clinical research protocols. Assists with patient educational activities Provides support to facilitate retention in protocols
Provides counseling to patients, families and significant others. Functions as a representative of the HIV Community Program and Christiana Care Health Services, interfacing with appropriate community and professional agencies
Acts as a resource person and assists in educational efforts involving HIV, with particular emphasis on psychosocial aspects. Precepts students at various levels as appropriate
Adheres to Christiana Care Health Services policy with regard to release of information, patient confidentiality and requests made by the news media. Discerns when confidential information should be shared in order to facilitate a multidisciplinary health care practice
Documents and maintains records in an appropriate, timely fashion. Demonstrates skill in providing documentation in current outpatient electronic medical record.
Demonstrates awareness of self-learning needs Interfaces with resource people in order to provide expert client care and facilitate self-development.
Represents Christiana Care Health Services and the HIV Community Program in a positive, professional manner.
Assists in data collection, maintenance of patient records, and quality assurance monitors when appropriate.
Demonstrates skills and knowledge necessary to provide care appropriate to adolescent, adult & geriatric , including knowledge of growth and development, the ability to obtain and interpret information to identify patient needs, and to provide the care needed.
Performs assigned work safely, adhering to established departmental safety rules and practices. Reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
Attends mandatory education programs annually Performs other related duties as required.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Bachelor's degree in Social Work
Licensed Bachelor in Social Work In Delaware
Demonstrated interest in HIV patient management
KNOWLEDGE, SKILL, AND ABILITY REQUIREMENTS:
Knowledge of Social Work Theory and understanding and acceptance of Social Work Code of Ethics
Knowledge of HIV disease process including epidemiology, treatment modalities, and research protocols
Knowledge of Crises Intervention Theory
Knowledge of Human Behavior and Developmental/Life Cycle Theory
Knowledge of community resources, state and federal programs
Skill in oral and written communication
Skill in interpersonal relations
Ability to relate to people like and different from self
Ability to communicate effectively with physicians, staff, patients, families as well as community and volunteer agencies
Ability to act as a role model in the maintenance and protection of patient confidentiality
Ability to assess when information should be shared with health care providers in an effort to facilitate patient care
