The Medical Foster Home Program Coordinator is a social worker who is responsible for maximizing psychosocial quality of life issues in order to assist veterans who need a nursing home level of care. In addition, this position is responsible for the administration of the Adult Day Health Care Program and the provision of social work case management services to the Veterans receiving care in Adult Day Health Care community sites funded by VHA. Basic Requirements: The basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C. § 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000. To qualify for appointment as a social worker in VHA, all applicants must meet the following: Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part). Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Applicants must meet the requirements for providing supervision in the state where they are licensed at the advanced practice level and should list these qualifications within the assessment narrative and their resume. Documentation is required if part of your state's requirements for providing supervision for licensure is beyond your licensure only (example: certification of continuing education credits). Grade Determinations: GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (document with written demonstration of KSAs must be included with application, see required documents section) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. The selection of medical foster homes and residents, evaluation and authorization for requests for Adult Day Healthcare, establishment of intake processes for both programs, provision of oversight responsibilities, and maintenance of a record-keeping system for both programs. Marketing the Medical foster home and Adult Day Healthcare model of care and the provision of community outreach to groups and agencies to promote both programs. Incumbent may serve as a mentor to other VA MFH program coordinators. Providing training to caregivers, students, residents, interns and fellows from other disciplines regarding the benefits/ limitations of a medical foster home and adult day health care. Have extensive knowledge of medical center policies and procedures, VHA and VBA regulations. The work may require assisting physically disabled veterans. Day-to-day work involves sitting, walking, standing and carrying light objects, such as medical records. Work is typically performed in a clinical setting that exposes the incumbent to the communicable diseases common of the client population. Some work is performed outside the medical center campus, where the environment cannot be controlled. The program requirement of the both ADHC and MFH requires on-site visits to the Medical Foster homes and ADHC sites in the community and therefore travel in a government vehicle is often required.
