ChristianaCare is seeking a Social Work/Case Management Assistant to support ChristianaCare case managers and social workers in assisting patients and their families to recognize and utilize internal and external resources to resolve social, emotional, and environmental needs by verifying benefits, obtaining insurance authorizations, calling in referrals and orders to vendors and accurately entering data into clinical information systems. The successful candidate will work Monday through Friday 10am to 6:30pm. Weekends and holidays are required.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Assists with copying, faxing, form completion, and letters.
Assists with completion of applications for community services and financial eligibility such as DAST, Long Term Care Medicaid, etc. Includes assisting to obtain verification(s) needed to complete the eligibility process.
Assists in contacting community agencies as to availability of services and eligibility requirements, i.e. maintaining nursing home portal.
Assists with routine transportation arrangements and obtaining clothing from departmental/community clothes closets for patient discharge.
Establishes collaborative relationships with care managers/ social workers/vendors/on-site liaisons and other health care staff to effectively and efficiently complete necessary services.
Enters accurate data, authorization/referral entries into computer systems as directed by team in an accurate and timely manner.
Call insurance companies for authorization, benefit verification and coverage information, as necessary.
EDUCATION AND EXPERIENCE REQUIREMENTS:
High School diploma or equivalent required.
Bachelor degree in social work or related field/behavioral sciences preferred.
Three years of experience in a support role in care management will be considered equivalent to an associate degree in social work or related field/behavioral sciences preferred.
Home health care and/ or managed care experience preferred.
