The Medical Supervisory Social Worker will function in both a clinical and administrative capacity. The incumbent will provide 70% direct acute medical social work services to Veterans, and provide 30% administrative oversight to those acute inpatient medical social work staff at University Drive. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensed or certified by a state to independently practice social work at the master's degree level. In addition to the basic requirements stated above candidates must also meet the below requirements per grade level listed below: Grade Determinations: Supervisory Social Worker, GS-12 (1) Education and Experience. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. (2) Licensure/Certification. Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. Candidates must demonstrate all of the following KSAs: (a) Ability to independently organize work, set priorities, and meet multiple deadlines. (b) Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement. (c) Ability to ensure provision of clinical social work services. (d) Ability to provide staff training and development. (e) Skill in interpersonal relationships in dealing with employees, team leaders, and managers. Provide liaison and referral services to community agencies and VA services. Provide care coordination service for patient/families during admission, ambulatory setting and ensure home care is ordered by physicians when indicated. Maintains confidentiality and complies with professional ethics according to professional and Departmental standards. Completes all documentation as required in required timeframes. Completes all needed forms that assist with discharge under direction of physician. Coordinate community-based services, including information and referral for additional services from other VA Programs, other government programs and community agency programs. Provides education on Advance Directive, End of Life, Palliative and Hospice Care issues. Assesses patient's/family's adaptation to patient illness and ability to provide for patient's care needs and provide resources in areas that are identified as areas for improvement. Serves as a resource to the interdisciplinary team for community based social service agencies and VA programs. Member of the Interdisciplinary Team. Attend treatment team rounds and actively participate in regards to psychosocial issues and discharge needs. Reviews patient information for assigned caseload, determines anticipated length of stay and discharge plan and begins discharge planning process in collaboration with the interdisciplinary team, patient, family. Interacts with home care staff, community agencies and facilities to ensure safe and timely discharge. Negotiates with interdisciplinary team follow up contact with patient/family, community agency or facility to evaluate the effectiveness of the discharge plans and identifies problems in service delivery.
