The incumbent functions as a LPMHC on a community-based interdisciplinary team. The incumbent serves a caseload of up to 10 Veterans who experience severe and persistent mental illness, including Schizophrenia and Major Affective disorders, who have frequent or long-term hospitalizations and who live in the community. Program services include frequent home visits, providing medication monitoring, crisis/stress management, and to assist with developing linkages to community resources. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
