Overview
Atlantic Health System is seeking a full-time Social Worker for Case Management Department.
Responsibilities
Atlantic Health System is seeking a Full-time Social Worker at Overlook Medical Center.
- Dual position split between acute care setting and family practice medical setting
1.Inpatient SW roles and responsibilities include but not limited to:
Adjustment to illness
Psychosocial evaluation
Transitions of care
Works as consultant with and to healthcare team
Knowledge of payer systems and resources
Assists with complex patients and family systems
Supportive counseling
2. Family Medicine practice setting includes but not limited to:
Spanish language required /diverse population /bi-cultural
Integration of SW support services in primary care setting
Barriers to Medical care and Social determinants of health
PHQ for depression screening
Substance abuse
Behavioral health needs /resources
Safety assessments including children adults and domestic violence
Capacity assessments
Suicide assessments
Evidenced based practices to help improve pt medical adherence and coping with illness.
Supportive counseling
Support role to CHW
Handles incoming calls /walk ins and Epic referrals from medical staff
Qualifications
EDUCATION/LICENSURE:
* MSW with current LSW or LCSW license in the State of New Jersey
Relevant Experience:
Bilingual Spanish speaking required : Bi-cultural Required
1 year previous medical / hospital SW experience and/or acute care experience preferred
( knowledge of systems- financial, insurance, advocacy, legal, and community resources.)
Familiarity with Epic preferred
