The Community Nursing Home Social Worker serves as a program associate coordinator at the GS-12 grade and assists with the administrative responsibility for the CNH program in the South Texas Healthcare System catchment area 75% of the incumbent's time will be dedicated to CNH. This position will also support the Community Care Network program (CCN) 25%. The incumbent assists with the development of policies and procedures for the program operations.
