Social workers assigned to Home Based Primary Care Team are responsible for providing direct patient care in the community (and via video telehealth) as a member of a multidisciplinary treatment team. In general, Home and Community Care serves a variety of patients located either in their own homes, who may be receiving HBPC (home based primary care) or AC (alternative care) services, or are living in a supportive setting such as a VA contract nursing home or Medical Foster Homes (MFH). Applicants pending the completion of education or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
