The Northwest Metro Primary Care Social Worker will coordinate with the Northwest Metro Primary Care Clinic teams to provide comprehensive medical social work interventions and care plans to Veterans served by this clinic. The social worker will provide on-site services. Incumbent works collaboratively as part of an interdisciplinary care team. Social Work supervision is provided by Primary Care Social Work CBOC supervisor at a location not adjacent to this clinic. Applicants pending the completion of education or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
