JOB SUMMARY: The Vanderbilt Home Care Social Worker 2, travels to the patients residence to provide psychosocial assessment of and brief targeted intervention for social, emotional and environmental issues that impact client/family's ability to optimally benefit from care. Actively collaborates with team to meet patient care management and system improvement goals. Represents the Social Work department on institutional and community committees/task forces. Provides leadership for department committees/workgroups. Contributes to the plan of care with knowledge of population specific needs as it relates to the identification of patient psychosocial needs.
Position Description:
The Social Worker 2 for Vanderbilt Home Care Services will be serving the home health population, doing 3 to 4 minimum home visits daily. Working closely with other home care clinical staff (Nurses, PT, OT, and SP). Must have Master's Degree and be a Licensed Master Social Worker.
Sign On Bonus: $5000 (paid out after 60 days of employment) (Prior employees must be separated from Vanderbilt for at least 1 full year to qualify for the bonus)

The Home Health Quality team is a small task oriented team which consists of a quality educator, a quality data analyst, and quality reviewer. They work closely together with clinical managers to provide meaningful data on home health patient population and clinician best practices.

KEY RESPONSIBILITIES:
Maintains professional documentation which meets department and VUMC standards as it relates to compliance.
Provides brief targeted interventions for identified clients.
Completes psychosocial assessment and assists team with the development of the plan of care.
Provides education and or consultation to clients and families, treatment team and community.
Contributes to the organization by actively participating in process improvement initiatives and shared decision making.
