This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going tohttp://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. ["The incumbent serves as a CC&ICM subject matter expert in partnership with nursing co-champion, providing guidance and direction to advance deployment and implementation of the CC&ICM framework. The incumbent is a lead member of the Care Coordination & Integrated Case Management (CCICM) initiative, serving as a lead coordinator for Veterans identified to benefit from intensive case management and care coordination services through the Cheyenne VA Medical Center. Additionally, the CCICM and Social Work Training Coordinator will serve on the Care Coordination Review Team (CCRT). The CCICM and Social Work Training Coordinator oversees the professional development of the social work employees/trainees employed at the Cheyenne VA medical Center. Additionally, the incumbent is responsible for the social work student internship and fellow program and management of its social work affiliates. The position is called upon to develop and present facility-wide multidisciplinary education on topics of expertise (i.e. professional boundaries, ethics, practice, etc.). The position also has oversight of the Advanced Clinical Licensure Supervision Program, including oversight of staff providing clinical supervision toward licensure. CCICM duties include: - Executes leadership in care coordination and integrated case management in collaboration with CC&ICM VISN and National leadership and key stakeholder program offices. - Collaborates with CC&ICM national leadership in the development and implementation of policies, practices, and standards of care coordination in alignment with VHA Directive 1110.04, Integrated Case Management Standards of Practice. - Analyzes and identifies barriers to CC&ICM implementation, and in collaboration with VA facilities within the VISN, develops proposed solutions to minimize or eliminate them and improve overall CC&ICM operations. - Implements effective collaboration, communication, and coordination approaches across the network and up to VHA Program Offices as it relates to care coordination services. - Ensures that facility adheres to standards that align CC&ICM services into an integrated, Veteran-centered framework. - Leads and promotes a consistently high performing facility through the establishment and use of standardized patient complexity evaluation instrument(s), standardized level of care coordination stratification methodology, and care coordination/case management education and training curriculum. - Develops and maintains effective partnerships throughout other programs and clinical disciplines, both within the VA and in the private sector. - Actively participates in regular meetings with CC&ICM National and VISN leadership as well as VA facility teams. - Promotes and disseminates promising and best practices through established communications (i.e., Community of Practice calls, email groups, electronic newsletters). - Ensure facility adheres to uniform standards of quality in care coordination and case management. Manages CC&ICM protocols and metrics, reports outcomes to VISN and VA facility leadership. - Identifies strategies and tactics to advance the VHA operational plan's goals and objectives applicable to CC&ICM. Social Work Training Program duties include: - The incumbent annually organizes the recruitment of graduate social work interns and fellows, manages the selection process, meets deadlines for submission of information internally and externally to collaborating Universities. Prioritizes training experiences such as rotations and didactics for interns, and assigns specific duties to members of the Training Committee, and other training leadership. - Skill in managing and directing the work of others to accomplish program goals and objectives, and ability to devise ways to adapt work operations to new and changing programs, staffing, and budget requirements as stipulated by the Office of Academic Affiliations (OAA), recruits staff to serve as rotation supervisors, and oversees quality management and evaluation of training program generally. - The incumbent is responsible for insuring that social work interns and fellows can meet their training goals as well as the treatment goals of the medical center in such a manner as is consistent with medical center rules and regulations, national VA Handbook regulations, State Licensing Board(s) for Social Work, local service operating procedures, and overarching NASW guidelines and code of ethics.
