The Senior Social Worker will serve a dual role as the Women's Health Clinic (WHC) Patient Aligned Care Team (PACT) Social Worker and as the Lesbian, Gay, Bisexual, Transgender, and Queer (LGBTQ+) Veteran Care Coordinator (VCC) at the Long Beach VA Facility. Incumbent is an integral member of a comprehensive team which delivers primary care to Veteran patients with its focus on prevention, health promotion, care coordination and chronic disease management.
