The Social Worker (Program Coordinator) serves as the Social Work Service, (SWS) Program Coordinator for both the Choose Home/Homemaker Home Health Aide program as well as the Veteran-Directed Home and Community Based Services (VD-HCBS) at the Overton Brooks VA Medical Center, Shreveport, LA. The Veteran-Directed Home and Community Based Services (VD-HCBS) Program is one of VHA's new and innovative programs that promote Patient-centered, non-institutional long-term care.
