The Caregiver Support Program (CSP) Social Worker for the Program of Comprehensive Assistance for Family Caregivers (PCAFC)/Program of General Caregiver Support Services (PGCSS) assigned to the CSP with responsibility to work closely with Caregiver Support Program Leadership and/or designee to complete program required home visits, telephone contact, assessments, annual evaluations, and respond to Caregiver Hotline calls.
