The Primary Care Social Worker provides social work services at the medical centers and works within a Patient Aligned Care Team (PACT). They are members of the health care team and assess and treat the psychosocial problems that impact on the ability of the veteran to make adjustments to his health care issues. The primary care social worker has a broad and comprehensive knowledge of VA and community resources and uses a variety of interventions in the amelioration of medical conditions.
