Travel to skilled nursing facilities and assisted living facilities
Will consider part-time or full-time flex schedule
Develop and follow a treatment plan to meet the needs of patients
Perform written or verbal consultation with the primary care physician, family, and facility staff
Document treatment plans, psychotherapy sessions, and treatment plan
Meet with the resident care coordinator or other staff as designated by the facility
Establish and maintain open and positive communications with facility staff and administration
