This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-12 Social Worker (Program Coordinator) Experience/Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs. Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Experience in identifying and engaging community resources for patient care Experience in working as a member of interdisciplinary team including medical, mental health and rehabilitation providers Familiarity with the cognitive, behavioral, and psychosocial challenges of patients with traumatic brain injury and other mental health and medical disorders such as post-traumatic stress disorder, depression, substance abuse and chronic pain Driver's License: Possess current driver's license and drive a government vehicle in carrying out professional duties when deemed necessary. This Position involves periodically transporting patients and requires a valid driver's license. The Social Work Program Coordinator, GS-0185-12, provides clinical and administrative oversight to a specialty program, managing the daily operation, developing policies and procedures and preparing reports and statistics for facility, VISN and national use. The Homeless Grant & Per Diem Program is an essential part of VHA and is vital for providing safe transitional housing and supportive services for homeless veterans. The Homeless Grant & Per Diem Liaison is responsible for the oversight of these services as well as the case management and service coordination. Duties for the position include but are not limited to: Administers information and analytical systems to evaluate and enhance the quality of services provided to patients in the Grant & Per Diem housing program. Develops and implements information systems to track service activities including visits, referrals, length of stay, patient demographics, equipment utilization, patient satisfaction, discharge outcomes and homeless performance measure success. Develops, implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Provides case management services at Homeless Grant & Per Diem sites, utilizing various treatment modalities of individual and group work, casework, community organization, and community outreach activities to alleviate or remove problems that interfere with the Homeless Grant & Per Diem Program participants' ability to achieve independence. Provides clinical consultation to staff at Homeless Grant & Per Diem sites. Monitors the clinical care and assesses the compliance of the programs receiving per diem as described in the original grant application. When necessary, intervenes to facilitate compliance or corrective actions and, when appropriate, develops, facilitates, and monitors plans of actions. Coordinates and directs annual inspections at all Grant & Per Diem sites.
