HealthVine is a Pediatric Accountable Care Organization. Our HealthVine Team is tasked with enhancing and improving the health outcomes and cost of care for all children with CareSource Medicaid insurance in the eight southwest counties in Ohio. Our mission is to coordinate care and support services for families that enable children to thrive by living their healthiest lives.
By coordinating community resources on behalf of patients and families, we are re-imagining how we deliver high-quality care to more kids in our region so they can have the best outcomes, experience, and value. HealthVine's broad influence includes work with families, payors, community benefit organizations, primary care clinics (both Cincinnati Children's Primary Care and community primary care), schools, subspecialists at Cincinnati Children's, as well as local adult hospital systems that care for pediatric patients and our long-standing Provider Hospital Organization (PHO), Tri-State Child Health Services.
We are currently recruiting for a full-time Social Work Care Manager to join our HealthVine Team. Responsibilities of the Care Manager include:
Conduct comprehensive psychosocial assessment of the patient's/family's health and needs
Partner with the patient/family/caregivers to obtain financial assistance, community resources, and/or specialized equipment
Provide psychosocial service options as identified in the patient's comprehensive plan of care
Promote patient/family/caregiver well-being
Facilitate communication and coordination between members of the health care team (including the medical home and community services), involving the patient/family in the decision-making process in order to minimize fragmentation in the services
Ideal candidates will have a master's degree in Social Work, and be a Licensed Independent Social Worker (LISW) in the State of Ohio. Care coordination experience preferred. Candidates must also have 5-7 years of related work experience.
Benefits begin immediately. In addition to health plans, we offer tuition reimbursement, paid time off, and a generous fully funded pension plan, among other great benefits. The pay range for this position is $69,000-$87,000.

Qualifications
EDUCATION/EXPERIENCE
Required:
Master's degree in Social Work + LISW in the State of Ohio
5 - 7 years of work experience in a related job discipline.
