Position responsible for participating in the coordination of all systems / services required for an organized, multidisciplinary team approach and assures quality, cost-effective care for the identified patient population.
Minimum Qualifications Education/Specialized Training/Licensure: Masters Degree in Behavioral Sciences or Social Work with Licensure (LPC or LMSW). An LCDC is preferred. Basic Life Support from a hospital-based American Heart Association (AHA) approved program.
Work Experience (Years and Area): 1 year experience in a hospital/health setting preferred
Experience working with patients with chemical use/abuse/dependency counseling and treatment preferred.

Special Requirements 1. Communication Skills: Above Average Verbal (Heavy Public Contract), Spanish preferred Writing /Composing (Reports )
