The incumbent in this position functions as the Case Manager for the Louisville VAMC's partnership with the Housing and Urban Development/VA Supportive Housing (HUD-VASH). Responsible for providing case management for a caseload of veterans in the HUD-VASH program. Services may be provided face to face or by telemental health at the medical center, CBOC, or in the community.
