The incumbent is the Rehabilitation Counselor assigned to the Health Care for Homeless Veterans (HCHV) Program. The incumbent provides outreach, assessments, referrals, and case management services to homeless veterans identified in the community. Services are provided within outpatient clinic, community, and transitional housing locations. The incumbent has expectations to meet the Veteran in the community to provide services with use of mobile technology, including employee safety devices. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
