The incumbent in this position functions as the community liaison; Outreach Social Worker for VA's Health Care for Homeless Veterans Programs. The incumbent is responsible for assertively pursuing unhoused Veterans in the VA CWM catchment, collaborating with various municipal, private, public, and other care-delivery organizations and teams in the identification, engagement of, and service delivery to unhoused Veterans. There are vacancies in Leeds and Worcester Massachusetts. Applicants pending the completion of educational, or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
