You will provide a wide range of services as part of a multidisciplinary team, with an emphasis on outpatient practice within the UCLA Primary Care Network. This will involve serving diverse child, adolescent, adult, and geriatric populations. You will be responsible for psychosocial assessments; care coordination; team consultation; individual, family, couples, or group psychotherapeutic treatment; serving as a liaison with community agencies; treatment plan documentation; and disposition planning and advocacy.

Salary range: $46.63-$55.73 Hourly
Qualifications
We’re seeking an experienced CA Licensed Clinical Social Worker with:
· Master’s degree · Experience collaborating with healthcare providers in a multidisciplinary setting · Working knowledge of general theoretical and clinical models · Ability to develop rapport with patients · Excellent interpersonal and communication skills · Ability to document/chart to ensure current and accurate medical records · Knowledge of social work ethics and scope of practice · Ability to perform a complete mental exam and recognize changes in mental status · Knowledge of relevant community resources · Proficiency in coordinating care with providers and community agencies · Ability to serve as a patient advocate with assertiveness, persistence, and diplomacy
